Print this document, obtain signatures, and mail the original to KBOR (Jean Redeker) at 1000 SW Jackson Street, Suite 520, Topeka, KS 66612, by July 15, 2009.

Performance Reports are due March 1, 2010 from all institutions.

The Kansas Board of Regents office will e-mail Performance Report forms.
